INTERESTED IN FREE CREMATION?

Consider tissue donation for high-end medical research. There are but a few accredited organ & tissue donor programs for high-end medical research.  Each program varies slightly in their donor acceptance parameters due in part, to the differing pools of research organizations with whom they each work. 

One item common to each of our donor programs; the family pays nothing, as there is No Charge to the family for the donation or subsequent cremation and timely return of the ashes.

Cremation Authorization

This a required form in which we are given specific instructions pertaining to the cremation/disposition of ashes and is signed by the legal next-of-kin.  If you will not be coming to the funeral home to sign this document please remember to also send a photocopy of a government issued Photo I.D. matching each signature.

Death Certificate

This is the form used to collect the information needed to file the original Death Certificate.  Please print out the form, type or print in the requested information.

Basic Obituary Form

This form will help you get a start on compiling basic obituary information for use in newspaper publications and funeral or memorial services.
